A Certificate of Origin is an export document required by foreign governments that validates the country of origin of where the product was manufactured. Farmingdale Chamber of Commerce offers Certificates of Origin for goods manufactured in the United States.
Farmingdale Chamber stamps Certificate of Origin forms as a benefit of membership (free of charge). This service is provided for members only. You must be a member in good standing (dues must be up-to-date) to receive this service. If you are not currently a chamber member, you can join here.
Download and complete a Certificate of Origin form and Affidavit. The affidavit releases the Chamber from any liability with regards to the goods. All Certificates of Origin and corresponding documents MUST be notarized before submitting to the chamber.
